How to prepare for your appointment:

Unison values your time, and we are doing our very best to make sure your experience with us is not disturbed by unnecessary waiting time, delays and miscommunication.

In return, we ask you to follow a simple set of recommendations that would benefit both the company and yourself.

  • Arrive 15-30 minutes prior to the appointment time
     

  • Bring a valid government issued ID for verification and documentation purposes
     

  • If you have any medical information to share with the doctor, please bring the Clinical Notes & Records to the appointment for review

  • Check your email and Client Portal updates to see if you have any initial documentation to complete prior to the appointment time
